Folic Acid Supplementation: A Foundation for Lifelong Health
Folic acid supplementation is a quiet powerhouse of ob-gyn health. This essential B vitamin helps the body create new cells such as blood and brain cells. It also performs a vital function during pregnancy: when used consistently before and during pregnancy, folic acid dramatically reduces the risk of neural tube defects in a fetus, turning a simple daily step into a game-changing act of protection.
How has folic acid supplementation developed over the years?
Identified in the early 20th century as a nutrient that helped prevent anemia, folic acid was synthesized in the 1940s for widespread use. By the late 20th century, research had linked folate deficiency in pregnancy to neural tube defects, including spina bifida and anencephaly, and major clinical studies had confirmed that supplementation before and during early pregnancy could dramatically reduce these risks.
In 1998, the United States required folic acid fortification of enriched grains, leading to a measurable decline in neural tube defects—a policy that remains a major public health success.
Why does folic acid supplementation matter?
Many people—especially people who may become pregnant—still don’t get enough folic acid through diet alone. Since early fetal development occurs in the period before most people know they’re pregnant, supplementation provides a reliable safety net.
ACOG recommends that most women of reproductive age take a daily supplement containing at least 400 mcg of folic acid to prevent neural tube defects, with exact dosing determined as part of the shared decision-making process between a patient and their ob-gyn.
What impact does this game changer have—and how can it shape the future of ob-gyn health?
Hear from Shilpa Mathew, MD, JD, FACOG, about how we can keep enhancing access to this simple, inexpensive, and effective intervention.
As we mark ACOG's 75th year, our Game Changers series will highlight breakthroughs that have reshaped ob-gyn care and explore what they mean for the future of health. Stay tuned—a new game changer will be featured each month!
